This role is only open to applicants that currently work within an NHS Trust within the Hampshire and Isle of Wight Integrated Care System (HIOW ICS).

Due to expansion of the services, we are looking for two new Cancer Support Workers within the Surgical Division, currently based in Urology and Colorectal. The successful candidates will work alongside Cancer Clinical Nurse Specialists to register and monitor patients through My Medical Records.

Main duties of the job

The main purpose of the Cancer Support Worker role is to support patients who are able to self-manage and to assist them in navigating the complex health and social care system during and after cancer treatment (Patient Initiated Follow Up (PIFU)/Patient Risk-Stratified Follow Up (PSFU)), using The My Medical Records (MMR) platform.

The post holder will be part of a wider network of Cancer Support Workers at HHFT but will work independently within the role's scope, under the guidance of the Clinical Nurse Specialist within the specific tumour group (e.g., breast, prostate, haematology, colorectal).
