Job overview

An exciting opportunity has arisen for a Band 7 Clinician to join a new Transfer of Care hub, coordinating care for Service Users who are in the most acute phase of a mental health crisis.

You will join a team that aims to use system knowledge and clinical skills to optimise each patient’s outcome and recovery. BNSSG Transfer of Care (TOC) hub is a fast-paced and ever-evolving environment. You will need to work flexibly and collaboratively within the TOC team, along with internal and external stakeholders.

We are looking for an enthusiastic, highly motivated, and dynamic individual who will work closely with the Crisis Teams to review admission requests and ensure Service Users receive the right care in the most appropriate setting, such as home treatment or inpatient admission.

The post holder will be required to attend and chair professional meetings regarding complex admissions where the pathway is unclear, to ensure the right care is offered at the right time and to facilitate flow across inpatient wards.

The post holder will escalate any high-risk cases or barriers to care in the community to the Service Manager in a timely manner.

They will play a pivotal role in managing flow during inpatient admissions to reduce delayed transfer of care cases.

Main duties of the job
  1. Review all inpatient bed requests clinically to ensure appropriateness.
  2. Line manage and work alongside bed coordinators to keep the daily bed management caseload updated with relevant clinical information.
  3. Identify service users' care needs as part of the MDT, make recommendations, and assess clinical risk as required.
  4. Bring together stakeholders with differing viewpoints to reach consensus on individual discharge plans.
  5. Lead the coordination of service user care in complex cases, admissions, or transfers, ensuring health and social care needs are fully assessed.
  6. Recognize potential problems and escalate to the service manager promptly.
  7. Understand all clinical TOC roles to deputize for colleagues as needed.
  8. Participate in health promotion activities, facilitate Inter Agency Meetings, and maintain the Inter Agency Network.
  9. Contribute to the development of policies and procedures within the TOC Hub.
  10. Maintain accurate records as per procedures, including electronic case notes, and attend training as necessary.
