Job overview

An exciting opportunity has arisen for a Band 7 Clinician to join a new Transfer of Care hub, coordinating care for Service Users who are in the most acute phase of a mental health crisis. The role involves overseeing a Service User’s journey from admission to discharge, ensuring a seamless and purposeful flow throughout this critical part of their treatment.

You will join a team that aims to use system knowledge and clinical skills to optimize each patient’s outcome and recovery. The BNSSG Transfer of Care (TOC) hub is a fast-paced and ever-evolving environment. You will need to work flexibly and collaboratively within the TOC team, along with internal and external stakeholders.

We are looking for an enthusiastic, highly motivated, and dynamic individual who will work closely with Crisis Teams to review admission requests and ensure Service Users receive the appropriate care, such as home treatment or inpatient admission.

The post holder will work closely with the out-of-trust placement manager to have clinical oversight of BNSSG service users receiving care outside of AWP.

Main duties of the job
  • Clinically oversee the care of service users in hospitals outside AWP.
  • Participate in daily bed calls to ensure out-of-area service users are linked with local services and discussed daily.
  • As part of the MDT, identify service users' care needs, make recommendations, and assess clinical risk as required.
  • Bring together stakeholders with differing viewpoints to reach consensus on individual discharge plans.
  • Lead the coordination of service user care in complex cases, discharges, or transfers, ensuring health and social care needs are fully assessed.
  • Recognize potential problems and initiate escalation to the service manager and Out-of-trust placement manager in a timely manner.
  • Build working relationships and understanding of roles within system partners.
  • Understand all clinical TOC roles to deputize for each other.
  • Contribute to the development of policies and procedures within the TOC Hub.
  • Maintain accurate records as per procedures, including electronic case notes, and attend training as necessary.
