A leading NHS organization is seeking a Band 7 Clinician for the Transfer of Care hub in Bristol. This role involves overseeing patient care during mental health crises, ensuring effective discharge planning, and collaborating with various healthcare teams. An exciting opportunity has arisen for a Band 7 Clinician to join a new Transfer of Care hub, coordinating care for Service Users in the most acute phase of a mental health crisis. The role involves overseeing a Service User's journey from admission to discharge, ensuring seamless and purposeful care flow.
You will join a team that uses system knowledge and clinical skills to optimize patient outcomes and recovery. The BNSSG Transfer of Care (TOC) hub is a fast-paced and evolving environment, requiring flexibility and collaboration with internal and external stakeholders.
We seek an enthusiastic, motivated, and dynamic individual to work closely with Crisis Teams to review admission requests and ensure Service Users receive appropriate care, such as home treatment or inpatient admission.
The post holder will work closely with the out-of-trust placement manager to have clinical oversight of BNSSG service users receiving care outside of AWP.

The post holder will coordinate care and discharge processes, supporting system flow and effective care by liaising with community teams and inpatient wards to ensure appropriate care and a safe, efficient discharge.
You will chair professional meetings regarding complex admissions, escalate high-risk cases or barriers promptly, and manage out-of-area flow during inpatient admissions to reduce delays. Effective discharge planning and multidisciplinary collaboration are essential.
The role requires a health or social care professional with mental health service experience. Building relationships with inpatient, community teams, private providers, and Approved Mental Health Professionals is vital, along with attending regular meetings to support system flow.
