• Manage caseload for Older Adults with varying mental health issues, co-morbidities, and complex presentations as a Keyworker
• Conduct assessments, reviews, interventions, and discharges, supporting relevant documentation
• Proficient in RiO documentation
• Complete multidisciplinary reviews in community and inpatient settings under the Mental Health Act
• Liaise with multidisciplinary team members and external agencies to support patient care
