• Manage caseload for Older Adults with mental health issues, co-morbidities, and complex presentations as a keyworker
• Conduct assessments, reviews, interventions, and discharges with relevant documentation
• Proficient in RiO documentation
• Complete multidisciplinary reviews in community and inpatient settings under Mental Health Act sections
• Liaise with multidisciplinary team and external agencies to support patient care
