Avon & Wiltshire Mental Health Partnership NHS Trust's FIND service is seeking a Registered Mental Health Nurse (RMN) or Registered Learning Disability Nurse (RNLD) to join the Forensic Intellectual, Neuropsychiatric & Developmental Disorders team.

You will coordinate a specialist caseload, undertake assessment, care planning, risk management and deliver evidence-based interventions to support recovery and safe community living.

Qualifikationen

• Must be RMN or RNLD with current NMC registration.

• Work as part of a multidisciplinary team to support individuals transitioning from secure/inpatient settings into the community, working with families, carers and partner services.
• Contribute to safeguarding and public protection processes and maintain accurate clinical records; support service development within FIND and related services.
