Qualifications

• Experience in risk assessment and Care Programme Approach.
• Current professional registration with NMC and up-to-date mandatory training.

Responsibilities

• Manage a defined caseload of patients with forensic histories and act as care co-ordinator and social supervisor as appropriate.
• Engage with MDTs and agencies to ensure high-quality, integrated care.
• Develop and maintain care plans, assess needs, monitor progress and review regularly.
• Organise admission, discharge and transfer processes in line with Trust Policies.
• Provide information to clients and carers about services, processes, and how to raise concerns.
