Job summary

CNTW NHS Foundation Trust in Sunderland is seeking a Band 6 Community Practitioner to join the Children’s Mental Health assessment and Treatment Pathway. You will coordinate assessments and deliver treatment for children and young people with complex mental health needs, working with families, schools, and multiagency partners.

Training and supervision will be provided. Applicants must be a registered health or social care professional with experience with children and families.

Qualifications

• Professional qualification and current registration as a nursing or allied health professional.
• Experience with children, young people and families.
• Knowledge of CPA and safeguarding practices.

Responsibilities

• Coordinate and contribute to mental health assessment and interventions within a multidisciplinary team.
• Offer evidence-based interventions to children and young people with complex needs.
• Provide clear, compassionate communication with parents, carers and professionals.
• Participate in MDT discussions and formulation meetings.
• Contribute to service development and quality improvement initiatives.
