Job summary

Savernake Hospital in the United Kingdom is recruiting a Band 5 Occupational Therapist to deliver rehabilitation and complex discharge planning for older patients. You will manage your own caseload within a multidisciplinary team and support therapy assistants and students.

Responsibilities

• Deliver evidence-based, patient-centred rehabilitation for older people
• Manage own caseload
• Plan and deliver complex discharge planning to support safe, timely discharges
• Assess, plan and deliver interventions that maximise patient independence
• Work collaboratively within the multidisciplinary team
• Communicate with patients, carers and colleagues
• Maintain accurate clinical records
• Recommend equipment where appropriate
• Ensure care meets professional standards and best practice
• Support therapy assistants and students
• Contribute to service improvement
• Maintain own continuing professional development
