Job summary

The Band 5 occupational therapist post is based on Jasmines Ward, an older people’s inpatient ward within the South West London and St George’s Mental Health NHS Trust. You will manage a defined caseload, deliver targeted OT interventions, and support recovery goals both in hospital and during home visits.

Key duties include discharge planning, collaboration with the multidisciplinary team, supervision of the activity coordinator, and contributing to care pathways from admission to discharge.

Qualifications

• HCPC registration to practise in the UK.
• Right to work in the UK.
• Fieldwork experience in physical and mental health settings.

Responsibilities

• Manage a defined caseload, assess, plan, deliver and evaluate interventions in hospital and on home visits.
• Act as the ward's Disability Champion, leading the multidisciplinary team on disability equality.
• Review client environment and functional ability using models and assessments.
• Carry out short-term OT to promote recovery and independence.
• Plan robust discharge and aftercare with clients and families.
