Job summary

Locala Health & Wellbeing is seeking a Band 5 Occupational Therapist to join the Intermediate Care Team in the community. This role is ideal for a newly qualified OT or someone looking to build experience in rehabilitation, reablement and community services.

You will manage your own caseload, perform holistic assessments in homes and communities, and deliver person-centred interventions to promote recovery, independence and wellbeing.

Responsibilities

• Assess individuals within their own homes and community settings.
• Develop and implement rehabilitation and reablement plans tailored to individual needs.
• Support safe discharge from hospital and help prevent unnecessary hospital admissions.
• Assess functional abilities and support people to achieve meaningful goals.
• Recommend and provide equipment and minor adaptations to maximise independence.
• Work collaboratively with nurses, physiotherapists, support workers, social care teams and other healthcare professionals.
• Support patients, families and carers to develop confidence and self-management skills.

About The Role
