An exciting opportunity has arisen for a Band 4 Discharge Coordinator to join our Hospital Complex Discharge Team. Working across Queen's Hospital and King George Hospital, you will support inpatient wards to plan safe, timely and appropriate discharges.
You will work closely with patients, families and carers, ward multidisciplinary teams, the Integrated Discharge Hub and community partners. You will help identify discharge needs and potential delays early, coordinate agreed actions, maintain accurate records and support patients who no longer meet the criteria to reside in an acute hospital bed.
We are looking for a motivated, organised and compassionate person with excellent communication skills. You should be able to prioritise your workload, work confidently with different teams, use digital systems accurately and remain professional in challenging situations. Experience in a patient-focused, customer-focused or social care environment is essential, together with an understanding of patient-centered care, hospital flow and multidisciplinary working.
This is a cross-site role and may include weekend working. You will be supported by ward teams, Discharge Leads and the wider Hospital Complex Discharge Team. Your work will contribute to earlier discharge, reduced length of stay, better use of hospital beds and improved patient experience.
Meet the patient within 24 hours of admission, or as soon as practically possible, to proactively gather relevant history and information, to identify risks or issues to inform the discharge planning process and pre-empt any actions required in advance, for example referral to therapies, homeless services, or social care.
Help identify potential delays to discharge on admission and in collaboration with the Multi-Disciplinary Team, record and monitor realistic Estimated Discharge Dates (EDD) to assist the Trust in meeting its NHS access targets.
Attend the clinical board rounds to be able to identify actions required to co-ordinate and facilitate discharge planning.
Identify discharges for that day and the following day and work towards same day/pre 12 discharges, prioritising actions that need to be taken by the multidisciplinary team to deliver this.
We're an organisation that is getting better and better and our improvements are driven by a determination to deliver care we're proud of and our patients are happy with.
They are benefitting from a new electronic patient record (we were the last acute trust in London to introduce one) and our maternity services have been rated good by the Care Quality Commission.
We operate from two main sites - KGH in Goodmayes and Queen's Hospital in Romford. We have two busy emergency departments with more than 346,000 people visiting them last year. We're campaigning to secure the £35m we need to transform the A&E at Queen's and get rid of corridor care.
We're proud of our regional Neurosciences Centre, Radiotherapy Centre and Hyper Acute Stroke Unit. We're also part of the North East London Cancer Alliance.
We run a Women's Health Hub in Ilford; an Ageing Well Centre in Hornchurch; and Community Diagnostic Centres (CDC) at Barking Community Hospital and at St George's Health and Wellbeing Hub. These CDCs are open 12 hours a day, 7 days a week.
The majority of our 8,400 staff - who come from 146 different countries - live in the three diverse London boroughs we serve and are from black, Asian and minority ethnic groups. Many can work flexibly and more than 400 of them are on our Ofsted accredited apprenticeship programmes. We're proud to be a London Living Wage employer.
