HealthHero Integrated Care is seeking independent Prescribing Clinicians to support urgent care services during out-of-hours in Shropshire, Telford and Wrekin. Roles include in-centre, home-visit and triage shifts with senior clinician support remotely.

We value autonomous practitioners with prescribing qualifications, MSc in Advanced Practice, and experience across adult and paediatric care, triage, and emergency pathways. Flexible contracts available in Telford HQ and regional centres.

Qualifikationen

• Registered nurse, paramedic, or allied health professional with non-medical prescribing qualification.
• At least two years prescribing experience.
• Ability to work autonomously both in-hours and out-of-hours.
• Experience in physical assessment and clinical reasoning for patients of all ages.
• Understanding of emergency care pathways and primary health care issues.

Aufgaben

• Autonomous clinical assessment, diagnosis, and treatment for adults and paediatric patients attending centres or during remote consultations.
• Accurate triage and prioritisation using diverse assessment techniques.
• Undertaking home visits with a fully equipped medical car and driver.
• Refer patients to appropriate care settings as needed.
• Support Care Coordination Service from the regional Head Office.
• Provide triage of NHS111 referrals during core hours.
• Support other clinicians on shift as required.
