Job summary

HealthHero Integrated Care operates urgent and out-of-hours healthcare services across the Shropshire, Telford and Wrekin locality. Independent Prescribing Clinicians will work from the Telford Headquarters or primary care centres, with shifts including face-to-face, home visits, and triage at the regional Head Office.

You will assess, diagnose, treat, and discharge or refer patients, triaging effectively, and supporting care coordination.

Qualifications

• Registered nurse, paramedic, or allied health professional with non-medical prescribing.
• At least 2 years prescribing experience.
• Experience with clinical assessment and paediatrics.
• Able to work autonomously in hours and out of hours.

Responsibilities

• Perform clinical assessment, diagnosis, treatment and discharge/referral for adults and paediatric patients.
• Accurately triage and prioritise patients using diverse assessment techniques.
• Undertake home visits with a fully equipped medical car and driver.
• Refer patients to appropriate care settings when needed.
• Support Care Coordination from regional Head Office.
• Triage NHS111 pathway calls during daytime hours.
