Shropshire Community Health NHS Trust is recruiting for Registered Nurses with experience in a multi-disciplinary health and social care setting. You will conduct holistic assessments, prioritize care, and make decisions under pressure within a patient-centred approach.

Role involves delivering high-quality nursing care, advocating for patients, and coordinating referrals while maintaining accurate records in line with NMC guidelines.

Qualificações

• Registered Nurse with current NMC registration.
• Experience in multi-disciplinary / multi-agency health and social care environments.
• Ability to perform holistic nursing assessments and make decisions under pressure.
• IT literacy including Microsoft Word and Excel.

Responsabilidades

• Formulate and deliver high quality patient care without direct supervision, seeking guidance on actions that are outside agreed defined standards.
• Recognise changes in patients' conditions which require the intervention of others and ensure timely referrals or escalation procedures.
• Ensure patient safety is maintained at all times. Record and report adverse events and assist investigations, sharing learning with the wider teams.
• Maintain clear, accurate and contemporaneous records in line with current NMC guidelines.
• Access and use patient clinical information to inform clinical decisions.
• Make appropriate referrals to members of the multi-disciplinary team and external agencies to meet holistic needs.
• Ensure high standards of nursing care and act when standards are not maintained.
• Act as an advocate for patients, challenging any barriers to their opinions.
