Hywel Dda University Health Board is recruiting a Specialist Physiotherapist for Cardiac Rehabilitation in Ceredigion. The post holder will work autonomously, providing specialist assessment and treatment with a caseload across community and Canolfan Rheidol, Aberystwyth.

You will lead group and individual exercise sessions, develop home exercise programmes, and use outcome measures to monitor progress. The role supports bilingual service delivery and ongoing professional development.

Formación

• Degree in Physiotherapy
• Current HCPC registration
• Postgraduate qualification or evidence of study at postgraduate level
• Demonstrable specialist knowledge relevant to the clinical area
• Evidence of relevant post-registration continuing professional development
• Membership of the Chartered Society of Physiotherapy (CSP)
• Active involvement in specialist professional groups

Responsabilidades

• Provide a comprehensive assessment of patients with diverse or complex presentations and devise an evidence-based treatment plan.
• Communicate with and work alongside nurses, a secretary, MDT members and other community services.
• Lead group and individual exercise sessions and provide home exercise programmes.
• Use validated outcome measures and risk assessments to monitor progress and optimise outcomes.
• Complete electronic information and discharge summaries and relevant database entries for audit and evaluation.
