Responsibilities

• Provide autonomous assessment, diagnosis and management of patients with undifferentiated, acute and chronic conditions.
• Undertake telephone, video and face-to-face consultations as clinically appropriate.
• Independently prescribe within professional competence and current legislation.
• Refer appropriately to secondary care, community services and the wider multidisciplinary team.
• Deliver health promotion, disease prevention and self-management advice.
• Undertake home visits where clinically appropriate.
