Job summary

Fenland Primary Care Network (PCN) is recruiting an Advanced Clinical Practitioner/Paramedic under ARRS to work across four member practices. The role delivers continuity of care through in-surgery clinics and home visits, accountable to the PCN and referring GP practices, and supports multi-disciplinary teams across the Fenland region.

You will join a collaborative team, bring advanced clinical skills, and help reduce unnecessary hospital conveyance by timely interventions and onward referrals

Qualifications

• Demonstrable experience working in primary care environments.
• Comprehensive patient examination skills.
• Knowledge of common acute and chronic conditions likely to be seen in primary care.
• Collaborative working and building relationships across organisations.
• Facilitating change to improve clinical practice; policy review and clinical audit.
• Understanding of clinical governance and quality issues.

Responsibilities

• Assess, plan, and deliver care in line with the patient's individual needs.
• Undertake in-surgery clinics and home visits as directed.
• Provide education to service users and empower self-care.
• Work with MDTs across PCN and connect with other health and social care agencies.
• Support reduction of unnecessary hospital conveyance and admissions.
• Maintain accurate healthcare records and participate in PCN MDTs.
