Job summary

South Rural PCN is recruiting an experienced Paramedic/FCP to join the Holbrook and Shotley practice. The role is full-time, with part-time considered for the right candidate, and is open to those with prior primary care experience.

Requirements include HCPC registration, primary care experience, and a two-year Consolidation of Learning, plus three years at Band 6 level. Prescribing qualification is desirable.

Qualifications

Responsibilities

• Respond to referrals and assess, diagnose and treat at first contact.
• Attend patients in practice or at home and conduct housebound rounds.
• Support on-the-day emergency patient services in the practice.
• Triage and prioritise patients using clinical history and examinations.
• Initiate treatment/referral and review effectiveness to improve outcomes.
• Maintain accurate clinical records efficiently.

About the Role

South Rural Primary Care Network, are looking to recruit an experienced Paramedic/FCP to join their team based at the Holbrook and Shotley practice. This role is only open to applicants with previous primary care experience. This position is offered on a full-time basis but part-time can be considered for the correct applicant.

Please note, at present the PCN and practice cannot support the ACP pathway, this position is for a FCP pathway Paramedic.

• Urgently respond to referrals and provide assessment, diagnosis and treatment at first point of contact by attending to patients, according to patients’ needs either in the practice or patient homes.


• Visit housebound and care home patients and complete care home ward rounds.


• Support, and continue the development of, an on-the-day emergency patient service in the practice.


• Accurately triage and prioritise patients, including in both emergency situations and telephone triage, demonstrating the use of a variety of techniques to elicit the history of an event/illness, including past medical and drug history.


• Evaluate clinical information from examination and history taking, and initiate appropriate treatment and/or referral.


• Review the effectiveness of the treatment provided, making changes where necessary to improve patient outcomes.


• Maintain accurate clinical records in an efficient manner.

About the Candidate

You will have the following skills and experience:

• Be a registered paramedic, with current HCPC registration

• Has completed their two-year ‘Consolidation of Learning’ period as a \"newly qualified paramedic\"


• Has a further three years’ experience as a band 6 (or equivalent) paramedic


• Prescribing qualification desirable

South Rural PCN provides services to 56,000 patients across rural Suffolk and is headed by 2 proactive, supportive and committed Clinical Directors. Our PCN includes the following practices: Bildeston Health Centre, Constable Country Medical Practice (East Bergholt and Capel St Mary), Hadleigh Health Centre, The Holbrook and Shotley Surgeries and Needham Market Country Practice.

Bildeston Health Centre is a friendly, medium sized, rural dispensing practice. We are CQC rated GOOD and score very highly on the Patient Experience survey. We have a growing list size of 7,600+ patients with two GP partners. We currently have 2 practices nurses, 2 HCAs, and a very supportive care navigation and admin team.

We are active members of the South Rural PCN who provide us with a practice paramedic and clinical pharmacist.
