Maidenhead Primary Care Network in the United Kingdom seeks an experienced clinician to deliver high‑quality patient care within a collaborative PCN. You will apply advanced, autonomous clinical skills and a broad theoretical knowledge base to assess, diagnose, and manage a wide range of conditions.

The role includes triage, home visits, care planning, and contributing to Same Day Access Clinic sessions. You will lead and train other healthcare staff, prescribing in line with guidelines, and

Responsabilidades

• Deliver high-standard patient care with autonomous clinical skills and broad knowledge.
• Assess, diagnose and treat patients, arranging referrals and planning care.
• Perform safe triage via telephone or face to face.
• Undertake home visits for assessment, diagnosis, treatment and ongoing management.
• Provide care home visits with assessment and ongoing management for registered patients.
• Deliver sessions in the PCN Same Day Access Clinic for acute presentations.
• Prescribe medications in line with evidence-based practice and protocols.
• Provide information on medications, side-effects and interactions.
• Prioritise health problems and intervene in urgent or emergency situations.
• Provide leadership, education and training to other healthcare professionals and staff.
