Job summary

Maidenhead Primary Care Network in the United Kingdom seeks an experienced clinician to deliver high‑quality patient care within a collaborative PCN. You will apply advanced, autonomous clinical skills and a broad theoretical knowledge base to assess, diagnose, and manage a wide range of conditions.

Responsibilities

• Deliver high-standard patient care with autonomous clinical skills and broad knowledge.

• Perform safe triage via telephone or face to face.
• Undertake home visits for assessment, diagnosis, treatment and ongoing management.
• Provide care home visits with assessment and ongoing management for registered patients.
• Deliver sessions in the PCN Same Day Access Clinic for acute presentations.
• Prescribe medications in line with evidence-based practice and protocols.
• Provide information on medications, side-effects and interactions.
• Prioritise health problems and intervene in urgent or emergency situations.
• Provide leadership, education and training to other healthcare professionals and staff.

Responsibilities

• Delivering a high standard of patient care, using advanced, autonomous clinical skills and a broad in-depth theoretical knowledge-base.
• Assessing, diagnosing and treating patients with both minor and complex needs, initiating referrals as appropriate and planning clinical care accordingly.
• Performing safe triage of patients when necessary via telephone or face to face.
• Undertake home visits for the practice and the PCN to provide timely assessment, diagnosis, treatment, and ongoing management for patients identified through the networks practices.
• Undertake care home visits for registered patients of the base practice, providing assessment, diagnosis, treatment, care planning and ongoing clinical management as appropriate.
• Deliver clinical sessions within the PCN Same Day Access Clinic, assessing and managing acute and urgent presentations within scope of practice.
• Prescribing medication appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
• Providing information and advice on prescribed or over-the-counter medication on medication regimens, side-effects and interactions.
• Prioritising health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
• Provide leadership, education and training to other healthcare professionals and staff.
