Job summary

Calderdale and Huddersfield NHS Foundation Trust invites applications for an experienced cardiology nurse to join our Heart Failure Specialist Nursing team as we expand the Hospital at Home service. The post is Band 6 with part-time, flexible working.

You will develop autonomy in heart failure management, benefit from learning opportunities such as a Heart Failure Diploma and Independent Non Medical Prescribing, and work with a supportive multidisciplinary team.

Qualifications

• First level registration NMC/AHP registered.
• Post-registration experience in the specialism or willingness to work towards.
• Teaching qualification.
• Non-medical prescriber or willingness to undertake.
• Cardiology experience essential.

Responsibilities

• Initial assessment and autonomous patient care in heart failure.
• Medication optimisation.
• Ongoing education and support to promote self-management.
• Review inpatients weekly at CRH and HRI as part of Hospital at Home service.
