Job summary

Calderdale and Huddersfield NHS Foundation Trust is seeking a Community Heart Failure Specialist Nurse (Band 6) for a part-time, permanent role with flexible working. The post involves 22.5 hours per week, pro rata salary of £39,959–£48,117 per annum, and the Hospital at Home service expansion across Calderdale and Huddersfield.

The role requires specialist cardiac nursing skills, non-medical prescribing, and the ability to work across multiple sites with a supportive multidisciplinary team.

Qualifications

• Post-registration experience/qualification relevant to the specialism or willing to work towards
• Teaching qualification
• Non-medical prescriber or willingness to undertake this

Responsibilities

• Initial assessment and ongoing education to promote self-management
• Medication optimisation and review for patients with heart failure
• Manage own caseload autonomously with high quality care
• One day per week dedicated to inpatient reviews at CRH/HRI and remote work when appropriate
