Job summary

Bradford Teaching Hospitals NHS Foundation Trust is seeking an experienced Heart Failure Nurse to assess patients, adjust treatment under local guidance, and work across clinics, wards and home visits to reduce hospital admissions.

The post requires a recognised teaching qualification, NMC registration, and a full driving licence with access to a vehicle. You will mentor staff, provide education, and support multidisciplinary team learning while delivering high-quality patient care.

Qualifications

• Registered Nurse
• Recognised teaching qualification
• NMC registration must be current and active.
• Significant cardiology experience is desirable.
• Experience with cross-boundary multidisciplinary teams and complex relationships is preferred.
• Commitment to continuous professional development may be expected.

Responsibilities

• Assess patients with heart failure across outpatient, inpatient and home settings
• Initiate and adjust heart failure medication and monitor outcomes
• Devise and review patient management plans under approved local and Trust protocols
• Work to reduce unplanned admissions and prolonged hospital stays
• Provide specialist advice to the multidisciplinary team
• Deliver education, training and clinical supervision to staff and service users
• Contribute to national and local heart failure priorities and service development
• Provide individualised care that supports patients and carers to take an active role
• Support the heart failure team with caseload management and triaging referrals
• Respond to enquiries from relatives and carers and act as patient advocate
• Assist in running the heart failure support group
• Exercise accountability under the NMC Code
• Understand information governance and confidentiality
• Evidence of continuous professional development
