Job summary

Kent and Medway Mental Health NHS Trust is seeking a Community Psychiatric Nurse to support Older Adults in Maidstone and manage a shared community caseload. You will work autonomously with peer support and will cover duty on a rota, ensuring safe, person-centred care in a fast-paced environment.

The role requires a full UK driving licence, resilience and strong assessment skills, with opportunities to contribute to NICE-guided practice within a multidisciplinary team.

Qualifications

• Valid UK driving licence and access to a car for business use.
• Carry out comprehensive nursing assessments considering mental, physical, social and environmental factors to create holistic care plans.
• Coordinate appropriate referrals and advocate for service engagement to maintain engagement.

Responsibilities

• Carry out and coordinate comprehensive nursing assessments and reviews to develop holistic, person-centred care plans.
• Facilitate referrals to primary and secondary mental health services and advocate for clients throughout the process.
• Lead and support best practice in line with NICE guidance.
