Job summary

The Central and North West London NHS Foundation Trust is seeking a Band 5 District Nurse to join the DN team in Hillingdon for an 8-month fixed-term period. You will visit patients in their homes, assess needs and deliver care with a focus on community-based outcomes.

You must be an NMC registered nurse, enthusiastic about autonomous working in the community, and prepared to work shifts including weekends. A literacy and numeracy assessment will be part of the shortlisting process.

Qualifications

• NMC registered nurse with Band 5 status is essential.
• Ability to work autonomously in a community setting.
• Experience in district/community nursing is desirable but not mandatory.

Responsibilities

• Provide district nursing care in patients' homes.
• Work within a multidisciplinary team to plan and deliver care.
• Contribute to delivery of care aligned with NHS strategies.
• Complete literacy and numeracy assessments as part of shortlisting.
• Support flexible shift patterns including weekends.
