Job summary

Mersey Care NHS Foundation Trust is seeking an enthusiastic Nurse Practitioner to join our Southport and Formby community teams. You will work autonomously to provide high standards of patient care, deliver evidence-based treatments including wound care assessment, injections, complete dopplers , compression bandaging and care planning, implementing evaluation and discharge of clients attending clinic.

The post holder would attend regular clinical supervision and encourage a culture of learning

Qualifications

• Awareness of Clinical Governance, patient group directions and policies.
• Understanding of audit processes and quality improvement.
• Excellent communication, organisational and interpersonal skills.
• Conflict resolution abilities within clinical settings.
• IT literacy and comfort with digital records.

Responsibilities

• Delivery of clinical care to a defined population under supervision.
• Provide wound care assessment, injections and doppler assessments.
• Develop and evaluate care plans and manage patient risk.
• Communicate with patients and carers using accessible language.
• Participate in clinical supervision, governance and continuous learning.
• Plan and prioritise workload in line with local guidelines.
