Qualifications

• NMC registration is essential.
• BSc (Hons) degree in 2:2 or above is required.
• Post graduate qualification is essential.
• Non-medical prescribing and CPD are required.
• Clinical skills course and evidence of diagnosing and arranging investigations are essential.
• Registered mentor status is required.

Responsibilities

• Comprehensive assessment of frail patients; formulate a clinical management plan.
• Coordinate care across primary, community and secondary care; support discharge planning.
• Prescribe safe, effective medications within statutory guidelines.
• Provide leadership and promote a collaborative, learning culture within teams.
