Job summary

Wirral Community Health and Care NHS Foundation Trust is recruiting a Community Staff Nurse (Band 5) for the New Brighton Community Nursing Team, Wallasey. This permanent, part-time post is 22.5 hours per week across 08:30-20:00 with weekend shifts. You will work within community settings, providing holistic care and long-term condition management.

Access to a car for work is essential and NHS terms apply. Pro rata salary £32,073–£39,043 per annum.

Qualifications

• Registered General Nurse educated to degree level (or equivalent).
• Current NMC registration.
• Evidence of further recent professional development.
• Mentorship module or working towards.

Responsibilities

• Provide holistic assessment and care planning for long-term conditions and frailty.
• Work across 7 days with shift flexibility.
• Maintain clinical standards using evidence-based practice.
