Job summary

Northumbria Healthcare NHS Foundation Trust is seeking a Community Nurse Practitioner (Band 7) on a fixed-term/secondment basis for 12 months. The role is 22.5 hours per week with flexible working patterns, including weekends and BH.

Based at Seaton Delaval, you will deliver evidence-based, clinically effective services in the Intermediate Care Units, supporting patients to remain at home where possible. You will lead autonomous clinical care, use non-medical prescribing skills, and coordinate

Qualifications

• NMC registration is required.
• Bachelor of Science (Hons) or above in nursing or a related field.
• Non-medical prescribing qualification.
• Clinical skills training with experience in assessment and investigations.
• Mentor registration or experience preferred.

Responsibilities

• Provide autonomous advanced clinical care in community and rehabilitation settings.
• Coordinate care for frail and acutely unwell patients across primary and secondary care interfaces.
• Prescribe safely within statutory guidelines and contribute to discharge planning.
