Job summary

Livewell Southwest is recruiting a Community Registered Nurse to coordinate holistic care in patients\' homes and community settings. You will assess, plan, implement and evaluate care, acting autonomously while supporting a Nurse Specialist in their absence.

The role emphasizes evidence-based practice, wound care, continence, palliative care, and MDT collaboration with a strong focus on professional development and governance.

Qualifications

• 1st Level Nurse Registration (degree or diploma) with NMC registration.
• Evidence of continuing professional development and HPC involvement.
• Experience in community nursing and wound care management.

Responsibilities

• Assess, triage, plan, implement and evaluate care in patients' homes.
• Travel between Livewell Southwest sites and community locations.
• Maintain accurate confidential records and documentation.
• Support MDT discussions and provide patient advocacy.
• Contribute to governance, audits, and service development.
