Job summary

Cornwall Partnership NHS Foundation Trust is seeking highly motivated registered nurses to join the Penwith Community Nursing Teams based in Hayle or Penzance. You will work in patients' homes, supported by a multidisciplinary team, with opportunities to develop skills and knowledge while delivering person-centred care.

Shifts cover 8 am to 10 pm, 7 days a week; a valid driving licence and car are essential.

Qualifications

• Registered Nurse (Adult) with current UK registration (NMC).
• Recent relevant professional development is essential.
• Mentoring students or willingness to pursue mentoring qualifications.

Responsibilities

• Assess, plan, implement and evaluate care in patients' homes with minimal supervision.
• Collaborate with patients and carers to provide support and informed choice.
• Support the Community Nurse Team Leader and other locality teams.
• Work autonomously following organisational policies and guidelines.
