Job summary

Ash Grove Medical Centre is seeking an experienced Advanced Clinical Practitioner to join the team in Wakefield. This autonomous clinician will provide high-quality, patient-centred care across the practice, collaborating with GPs and the multidisciplinary team.

The ACP will conduct clinics, home visits and care home visits where needed, developing management plans and ensuring safe, evidence-based care for both adults and children where qualified.

Qualifications

• Qualified and registered healthcare professional with advanced practice credentials.
• MSc in Advanced Clinical Practice or equivalent qualification.
• Experience in autonomous clinical assessment and consultation.

Responsibilities

• Conduct autonomous face-to-face clinical consultations across the practice.
• Undertake home visits and care home visits as required.
• Assess, diagnose and manage a wide range of conditions within scope.
• Request investigations and interpret results within competence.
• Develop and review individualized treatment plans.
