Job summary

Bradford Student Health Service is a university GP practice in Bradford seeking an Advanced Clinical Practitioner to join the team. The ACP will work with patients autonomously in assessment, diagnosis and treatment, developing long‑term relationships and participating in multidisciplinary care.

ACPs in primary care can prescribe and manage a wide range of conditions, collaborating with both primary and secondary care teams.

Qualifications

• Autonomous practice in assessment, diagnosis and treatment.
• Work across primary and secondary care with multidisciplinary teams.
• Provide specialist advice and manage patients across all ages.
• Prescribing medicines where legislation allows.

Responsibilities

• Independently undertake clinical assessments, diagnose, treat and discharge or refer.
• Ensure patients are fully involved in the design and implementation of their treatment plans.
• Coordinate with multidisciplinary teams to provide seamless care and prevent hospital admission.
• Refer appropriately to clinical leads/supervising GP when a second opinion is required.
• Adapt styles of communication to promote empowerment, self-management and health education for patients.
• Attend clinical case conferences and clinical meetings as required.
• Be a source of clinical expertise to less experienced staff within the team.
• Delegate duties/tasks to other members of staff where appropriate.
• Provide specialist advice relating to minor illness and/or injury to local healthcare teams, patients, relatives, and carers.
• Identify actual or potential clinical risks and take appropriate action.
