Job summary

Unicare Complex Care Specialist Ltd is seeking an experienced clinician to assess referrals, build care packages and stay through settling-in. You will work with the Registered Manager, report to the Clinical Director and oversee nurse-led and complex care packages across multiple settings.

You will hold a registered professional qualification and have substantial post-qualifying experience with autism, learning disabilities or mental health needs, plus the ability to travel across operating

Qualifications

• Structured assessment of need and risk.
• Excellent written English for commissioners.
• Experience setting up or embedding a care package.
• Knowledge of MCA 2005, Care Act 2014 and safeguarding.

Responsibilities

• Assess referrals and determine suitability for Unicare packages.
• Build and mobilise care packages with the Registered Manager.
• Provide ongoing clinical oversight during settling-in and sign-off stages.
