Job summary

Oxford Health NHS Foundation Trust is seeking an Advanced Clinical Practitioner to provide rapid response care at patients' homes, helping prevent hospital admissions. The role works across community nursing, therapy teams and primary care, with autonomous practice and multidisciplinary collaboration.

You will support patients with acute health and social care needs, manage long-term conditions, and contribute to service development within our community same-day care provision.

Qualifications

• Registered General Nurse or Allied Healthcare Professional incl paramedic.
• Post-registration qualification in advanced clinical assessment.
• V300 non-medical prescriber.
• Mentorship or teaching qualification or equivalent.
• Full UK driving licence and access to own vehicle.

Responsibilities

• Provide rapid response to health and social care crises to maintain independence and prevent hospital admission.
• Support management of long-term conditions with appropriate community links.
• Collaborate with Community Nursing, Therapy Teams, LAS, GP and hospital colleagues across community services.
• Work within a multidisciplinary team to ensure the right care is delivered by the right person at the right time.
