Job summary

The Bay Medical Practice in the United Kingdom seeks an Advanced Clinical Practitioner to join its multidisciplinary primary care team at an advanced level of practice. The role focuses on autonomous assessment and management of undifferentiated or complex cases, including home visits, with leadership, education and research elements embedded.

You will engage in prescribing where qualified, develop personalised treatment plans with shared decision-making, and contribute to service improvement

Qualifications

• Master's-level award in Advanced Clinical Practice or equivalent
• Independent prescribing qualification where prescribing forms part of the role
• Current registration with the appropriate UK healthcare regulator

Responsibilities

• Assess and manage patients at the practice and in their home, including reaching diagnoses and treatment decisions
• Manage clinical complexity, risk and deterioration, escalating urgently when needed
• Prescribe independently where qualified, registered and authorised
• Develop and review personalised treatment plans with shared decision-making and safety-netting
• Request, interpret and act on investigations within competence and local pathways
• Refer to and work with GPs and community, hospital, pharmacy, mental health and social care services
• Provide clinical leadership and support quality improvement and safe service development
• Support the education, supervision and development of colleagues and learners
• Use evidence, audit and data to improve care and reduce unwarranted variation
• Keep an up-to-date advanced practice portfolio and maintain professional development
• Keep accurate clinical records of assessment, decisions and follow-up
• Follow safeguarding, infection control, information governance and consent policies
