Job summary

London Central & West Unscheduled Care Collaborative (UC) is seeking an experienced Advanced Clinical Practitioner to join a multidisciplinary primary care team in the London area. The ACP will provide autonomous clinical assessment, diagnosis, treatment and management of a wide range of health conditions, delivering care via face-to-face, telephone and digital consultations.

The role involves working within an agreed scope of practice, prescribing appropriately, and collaborating with GPs,

Qualifications

• Substantial post-registration clinical experience.
• Experience as ACP or in advanced clinical role.
• Experience assessing, diagnosing and autonomously managing patients.
• Experience of independent prescribing.
• Experience with acute, chronic and complex cases.
• Experience in a multidisciplinary team.

Responsibilities

• Provide autonomous, evidence-based clinical care across acute, urgent and long-term needs.
• Conduct assessments, order and interpret investigations, and develop treatment plans.
• Prescribe medications and manage same-day urgent care.
• Collaborate with GPs, nurses, pharmacists and others to optimise care.
• Maintain accurate medical records and contribute to governance and quality improvement.
