Job summary

Beechfield Medical Centre in Lincolnshire seeks an Advanced Clinical Practitioner/Advanced Nurse Practitioner to join a growing team. The role focuses on assessment, treatment and signposting for minor illness within a Primary Care Network framework.

Responsibilities include on-site and home visits, patient education, and developing an on‑the‑day emergency service, with a strong emphasis on NICE guidelines and clinical governance.

Qualifications

• Registered practitioner with acute/minor illness management qualification and urgent care/GP setting experience.
• Experience in triage/initial assessment and autonomous practice.
• Knowledge of NICE guidelines and NHS professional codes (NMC/HPC).
• Experience with non-medical prescribing and PGDs.
• Strong communication and documentation skills.

Responsibilities

• Assess, diagnose, treat, refer or signpost patients with minor illness.
• Perform home visits for acute conditions as needed.
• Provide education to patients to promote self-care.
• Support on-day emergency patient service development.
• Maintain accurate documentation and patient records.
• Signpost to appropriate services and referral pathways.
