Job summary

Midlands Partnership NHS Foundation Trust is seeking an Advanced Clinical Practitioner to support Discharge to Assess (D2A) services at Haywood Community Hospital. The post holder will help ensure safe, timely discharges from acute and community settings, with ongoing assessment and intervention in the most appropriate environment.

Responsibilities

• Exercise advanced clinical expertise, levels of judgement, discretion and decision making in clinical care demonstrated through the HEE multi-professional framework for advanced clinical practice in England.
• Work closely with medical, nursing and AHP staff in both primary and secondary care, to ensure patients receive appropriate investigation, intervention and treatment with minimal avoidable delays, whilst improving the quality of care.
• Work autonomously as an advanced clinical practitioner encompassing the skills of advanced clinical assessment, initiating investigations, determine a differential diagnosis and initiating appropriate holistic, research-based treatment with the development of a personalised clinical management plan, including independent prescribing within their professional scope of practice.
• The post - holder will demonstrate safe, competent clinical decision making and expert care, demonstrating advanced clinical critical thinking to meet the needs of service users.
