HertsOne GP Federation is seeking a Mental Health Nurse prescriber to join the South West Herts Enhanced Primary Care Monitoring service. The role focuses on ADHD management in children and young people aged 6–18, including medication reviews, titration and monitoring for co-morbidities in a clinic-based setting.

The post involves working across 2–3 sites within West Herts and collaborating with GP supervisors, HPFT CYPMHS and other agencies.

Qualifikationen

• Current UK nursing registration (Mental Health).
• Experience in ADHD management in children and young people.
• Ability to liaise with families, schools, GP practices and multi-agency services.
• Understanding of safeguarding and child protection policies.

Aufgaben

• Provide holistic ADHD medication reviews and titration for children and young people up to age 18.
• Carry out clinic-based reviews and adjust drug treatment within agreed protocols.
• Communicate with GP supervisors, HPFT CYPMHS, families and other services; attend weekly MDT meetings.
• Prepare reports, care plans and referrals; monitor and document patient progress and side effects.
• Maintain accurate electronic and paper patient records and contribute to service development.
