Job summary

The post sits within the South West Herts Enhanced Primary Care Monitoring service run by HertsOne, a GP federation, as a 12-month fixed-term role. The nurse carries out medication reviews and holistic assessments for children and young people aged 6 to 18 with ADHD and co-occurring conditions, on referral from the local CYPMHS service.

The work is clinic-based across two or three sites, with weekly multidisciplinary meetings and autonomous medication adjustments within agreed protocols under GP

Qualifications

• Evidence of CPD is required.
• Safeguarding training to Level 3 is required.
• NMC registration as a mental health nurse is required.

Responsibilities

• Carry out nurse-led ADHD medication reviews in clinic, adjusting treatment within agreed protocols and NICE guidance.
• Holistic assessments of children and young people referred by CYPMHS.
• Titrate ADHD medication and monitor response and side effects.
• Identify co-morbidities and make onward referrals, raising complex cases with supervising GP.
• Attend MDT meetings and liaise with GPs, families, children's services and other agencies.
• Keep accurate clinical records and report data for audit and statistics.
