Job summary

Lancashire Teaching Hospitals is seeking a pathway navigator to support families through an autism assessment pathway. You will act as the central point of contact, coordinating communications and signposting resources to meet individual needs throughout the pathway.

The role involves collaborating with the community paediatrics team and NHS providers, identifying and resolving barriers to timely care, and contributing to audits and team meetings.

Qualifications

• GCSE English and Maths at grade C/4 or above.
• Experience working with autism or neurodevelopmental conditions.
• Experience handling sensitive information.
• Experience monitoring and auditing work.

Responsibilities

• Act as central contact for patients undergoing ASD assessment.
• Maintain communication with patients and signpost resources.
• Escalate concerns to relevant colleagues.
• Collaborate with the community paediatrics team and health providers.
• Identify barriers to care and delays.
• Coordinate access to support channels during the child’s assessment.
• Answer calls, respond to correspondence, and provide basic telephone advice.
