Job summary

Responsibilities

• Provide high-quality, person-centred support in the client’s home and community.
• Support daily routines and activities to promote independence.
• Encourage social engagement and emotional wellbeing.
• Build trusting relationships with the client and family.
• Follow care plans and safeguarding procedures; maintain records.
• Report any behaviours of concern per risk assessments.
