Durham County Council in Seaham, Durham, is recruiting a Social Worker to support adults with learning disabilities through strengths-based practice, helping them achieve independence, safety, and wellbeing within the community.

You will undertake assessments with individuals and carers, develop personalised care plans, coordinate with partners, lead safeguarding inquiries, and contribute to multi-disciplinary meetings, with a commitment to person-centred care and ongoing professional

Qualifications

• Recognised Social Work qualification and registration with Social Work England.
• Experience with adults, ideally within learning disability services.
• Knowledge of Care Act, Mental Capacity Act, and safeguarding responsibilities.

Responsabilités

• Undertake strengths-based assessments involving individuals and carers to identify needs and outcomes.
• Develop, implement, and review personalised care and support plans promoting independence and choice.
• Coordinate with partners and community resources to provide effective support and opportunities.
• Lead safeguarding enquiries and interventions in line with legislation and best practice.
• Monitor and review support arrangements for outcome-focused and responsive care.
• Contribute to multi-disciplinary meetings, advocating for individuals and ensuring their views are central.
• Maintain accurate, timely records and reports in line with professional standards.
