Job description

• Conduct thorough assessments of client needs and situations to determine appropriate assistance.
• Develop and implement individualized service and care plans, monitoring and evaluating client progress to modify plans accordingly.
• Provide counseling, support, and crisis intervention to clients facing various challenges.
• Advocate for clients to secure necessary services and resources, such as housing, food assistance, and healthcare.
• Coordinate and collaborate with multi-disciplinary teams, healthcare providers, schools, government agencies, and other community resources.
• Maintain accurate, confidential, and up-to-date documentation and client records, ensuring compliance with federal, state, and local legal requirements.
• Facilitate support groups and community outreach programs to promote social change and community well-being.
