NHS Tayside seeks a Primary Care Associate Practitioner to join their social prescribing team. This role involves managing a caseload of patients with complex needs to improve their health through non-medical interventions. Candidates should have SVQ 3 qualifications and excellent communication skills, ready to collaborate in a multidisciplinary setting.

NHS Tayside: Band 4, £29,116 - £31,670 per annum
DCC Grade 7: £27,511 - £32,084.02 per annum
Fixed term to 31/03/2026
This is an integrated post and the successful candidate can choose their employing body: NHS Tayside or Dundee City Council. One post is available; job share will be considered.
Sources of Support works alongside patients in Dundee GP Practices whose physical and mental health and wellbeing are impacted by social, economic issues, and personal circumstances. The service offers non-medical interventions and coordinated care to help improve patients’ quality of life, whilst alleviating pressures on practices due to non-medical issues.
An exciting opportunity has arisen within the service. We are looking for a highly skilled practitioner to join as a Primary Care Associate Practitioner (Social Prescribing). Our team includes 8 Primary Care Link Workers and 2 Primary Care Associate Practitioners (support workers). The post holder will work closely with the Primary Care Link Workers and with patients face-to-face in health centres and community settings.
The successful candidate will manage a caseload of up to 35 patients with complex needs, working with them for up to 12 weeks to help achieve their goals. They will support patients in accessing activities that promote health improvement, contribute to community-based interventions to improve life circumstances, support lifestyle changes, and address determinants of health and wellbeing using motivational interviewing.
This is a fast-paced role requiring excellent communication skills. Candidates must have experience working one-to-one with individuals facing complex social, economic, and personal circumstances. Experience in multidisciplinary and collaborative working with partners across various settings is also essential.
Applicants must be educated to SVQ 3 in a relevant area.
Internal applicants need to seek written approval from their line manager prior to applying for this secondment and must bring this approval if shortlisted for interview.
You will be required to achieve the mandatory Induction standards for Healthcare Support Workers in Scotland and to adhere to the Code of Conduct throughout employment.
