Southwark Council is seeking an Assistant Practitioner for their Community Review Team, focusing on supporting older adults and individuals with disabilities. The role involves assessments to determine service eligibility, crisis management, and collaboration with various professionals to promote independence and safeguard vulnerable adults.
We are recruiting for an Assistant Practitioner position within our Community Review Team (CRT).
The OPPD Service supports older adults and adults with physical disabilities. It is a multi-professional team comprising Social Workers, Occupational Therapists, and Assistant Practitioners.

We seek resilient practitioners capable of managing competing demands and prioritising effectively. You should have experience working with older people and individuals with disabilities, and be able to undertake strength-based social care assessments.
The role requires confident decision-makers who can remain calm under pressure. You should have experience collaborating with professionals and partner agencies, and be able to establish respectful, honest relationships with service users and their families or carers.
You will be able to deliver focused interventions to safeguard vulnerable adults, promote independence, positive outcomes, and the use of assistive technology.
