Role Overview

To provide a comprehensive and integrated social care pathway for Children and Young People with Disabilities and for adults with Learning Disabilities.

To ensure outcome-focused goals are achieved to increase or maintain independence and wellbeing.

To support Social Workers within the team, assisting in implementing support plans and helping service users achieve their goals.

Responsibilities include, but are not limited to:

1. Ensure statutory requirements for reviews are met for both Children’s and Adults, and undertake direct work with service users and their families/carers in line with support plans.
2. Identify through supervision when cases require further assessment and re-allocation to a social worker.
3. Promote access to paid employment, training, rehabilitation, social, and leisure opportunities to foster independence and social inclusion.
4. Support individuals in increasing independence within the home, including coaching on home care tasks, budgeting, and bill payments.
5. Work across agencies and professional boundaries for a holistic approach to assessment and intervention.
6. Assist people in maximizing independence and collaborate with colleagues and external professionals to support their potential.

Requirements:

• Knowledge of empowerment principles and promoting independence.
• Experience working in a person-centered manner to help service users achieve goals and maximize independence.
• Knowledge of relevant legislation, statutory guidance, and social work and care management practices, especially safeguarding.
• Experience supporting service users to identify and achieve their goals.
• Understanding of disabilities and their impact on individuals’ lives.
• Experience working with children, young people, and adults in various settings, including managing challenging behaviors.
• Awareness of issues affecting diverse communities.
• Enhanced DBS check within the last 12 months or willingness to obtain one.
